Bon Secours Community Hospital's Social Work Services Department is committed to promoting the total health and well-being of our patients. The department functions as a bridge between physicians, hospital services, the patient and their family, and provides arrangements for care beyond the hospital setting.

Social Services provides assistance in many areas including:

  • Helping the patient and their family understand the diagnosed illness and issues related to the illness.
     
  • Providing physicians with a better understanding of the patient's social and economic environment and how it impacts the patients medical care.
     
  • Putting patient and patient's family in contact with community resources, agencies, and health education programs which may be needed to provide continued care or promote good health.

Outpatients may contact the Social Work Services Department directly by calling 845-858-7122 or through their physician. Inpatients of Bon Secours Community Hospital may request a referral from their case manager, nurse, or physician.
